Support Coordination
Intellectual Disability Support Coordination provides case management services, as identified in a detailed Person Centered Plan, to assess the needs (medical, psychiatric, social, educational, residential, etc) of individuals with ID and coordinates, links, and monitors supports for qualifying individuals.

Horizons Day Support Program
Horizons is a center and community based program that serves ages 12 years through adulthood. This program provides supports, as identified in Individual Support Plans, to increase socialization, adaptive skills, communication, and more through the use of community resources (field trips, volunteer opportunities, etc) and center-based activities (arts and crafts, personal hygiene education, horticulture, etc.).

Sponsor Home Program
The Sponsor Home Program provides alternative living arrangements for individuals with an intellectual disability by providing them with a nurturing and supportive family home. Supports are provided as identified in an Individual Support Plan.

The place to start is ACCESS…
ACCESS is the entryway into any HCS program or service for individual consumers and referring agencies. Your call will be answered by a trained Access specialist who will ask you some simple questions and get basic information. They will then explain the service we provide to meet your needs, check with your insurance and schedule an initial appointment. ACCESS Number is 276.525.1550.
